Eastern Plumas Health Care is a small, non-profit, critical access hospital, providing comprehensive medical services to Plumas County since 1971. We operate an acute care hospital at our main Portola campus, which includes a 24-hour, physician staffed, emergency room and ambulance service. In addition, four primary care medical clinics and a dental clinic offer residents a full spectrum of health care services in the Graeagle, Greenville, Loyalton, and Portola communities. There are also hospital-based Skilled Nursing Facilities in Portola and Loyalton.

REPORTING RELATIONSHIPS:

The Licensed Clinical Psychologist (CP) or Licensed Clinical Social Worker (LCSW) reports directly to the Behavioral Health Program Manager and clinical supervision is provided by a supervising physician pursuant to their Collaborative Agreement.

POSITION SUMMARY:

The CP or LCSW, under the direction of the Medical Director and/or Behavioral Health Program Manager, is a self-directed individual requiring full participation and dedication within a self-directed team. The purpose of this position is to provide a variety of clinical services that include: initial intake; assessments; brief therapy, crisis evaluation; phone triage with clients; and, referrals to appropriate services. The CP or LCSW has the ability to work with patients and families who are experiencing a variety of psychiatric, medical and social problems utilizing individual, group and family counseling skills by providing direct therapy/counseling to clients to help them gain insight and understanding into issues adversely affecting their lives, such as mental illness, family or marital issues, psychosocial issues and issues related to addiction and abuse. This position will support primary care physicians in managing mutual patients.

PRINCIPAL ACCOUNTABILITIES:

1. Provides direct outpatient care to patients within the Behavioral Health unit of Eastern Plumas Health Care's outpatient, rural health clinics by utilizing evidence based therapeutic modalities.
2. Works in the capacity of an independent, licensed provider in a competent, capable, professional manner which exemplifies the current standard of care and practice in therapy.
3. Performs assessments on new and returning patients as required.
4. Assesses emotional, psychological and developmental status of patients utilizing a biopsychosocial approach.
5. Provides assessment to diagnose and treat mental health disorders and psychosocial disorders.
6. Develops and updates treatment plans to help patients meet their treatment goals following current standards of practice.
7. Initiates evidence based therapeutic treatment modalities and establishes treatment goals.
8. Identifies appropriate therapeutic treatment approach to help patients gain insight, understanding and identify issues and behaviors which adversely affect their lives, physical and mental health.
9. Participates in case conferences and team meetings to review individual needs and treatment plans.
10. Communicates and coordinates care with other providers as needed.
11. Communicates, when necessary, with the family and the patient in person, by letter or by telephone.
12. Communicates and coordinates care with outside referral sources on the care of mutual patients.
13. Assures the continuity of care for all patients in their care.
14. Provides other clinical duties as designated by the Behavioral Health Program Manager or Director of Outpatient Clinics or designee.
15. Additional duties as assigned.

EDUCATION/EXPERIENCE:

The following education and experience are required for the position:

• A doctorate degree in psychology from an accredited institution with a California psychology degree program approved by the American Psychological Association, or a master's degree in social work from an accredited university by the Council on Social Work Education.
• Completion of supervisory courses to supervise registered ASWs/Registered Psychologists.
• Must be licensed by the California Board of Psychology as a Psychologist or the California Department of Consumer Affairs, Board of Behavioral Sciences Examiners as a LCSW.
• LCSW who provides clinical supervision will have completed 15 contact hours of supervision training according the BBSE guidelines as required.

AND

• Must have a valid CA drivers license, have a good driving record and show proof of insurance coverage.
• BLS Healthcare provider for CPR required.

SKILLS:

• Strong clinical skills in psychosocial assessment, crisis intervention, counseling, interdisciplinary collaboration and linkage with resources.
• Knowledgeable in child, adolescent, adult and geriatric psychopathology as well as psychiatric interventions.
• Must be knowledgeable with psychiatric, psychosocial and legal issues and substance misuse/abuse and chemical dependency assessment and treatment, understanding of current insurance benefits and restrictions in the area of mental health, knowledgeable of DSM-IV TR, family system therapy and crisis intervention models.
• Knowledge of child, elder and dependent adult abuse and domestic violence reporting requirements and other significant regulations affecting clinical social work practice (e.g. Tarasoff, patient-psychotherapist privilege).
• Knowledge of suicidal and homicidal behavior and the skills necessary to assess lethality and to develop appropriate treatment plans.
• Knowledge and understanding of human behavior and communication, psychotherapy, case management and the influence of cultural and spiritual values in social work practice.
• Knowledge and skills necessary to provide psychosocial care appropriate to the age-specific needs of children, adolescents, adults and elders.
• Knowledge of individual and family development over the life spans.
• High level of expertise and knowledge of substance misuse and abuse and co-occurring disorders and the skills necessary to develop appropriate and successful treatment plans.
• Knowledge of recovery-oriented services, family systems and evidence based treatments (e.g. Cognitive and Dialectical Behavioral Therapy, Motivational Interviewing, Acceptance and Commitment Therapy, Mindfulness-Based Relapse Prevention, etc.)
• Demonstrate sound clinical judgment, problem solving and decision-making skills.
• Must demonstrate strong interpersonal and organizational skills to work effectively in a fast-paced environment with rapidly changing priorities and competing demands.
• Ability to establish and maintain effective working relationships with others.
• Ability to handle difficult situations and to control client behavior in emergency situations.
